CODI Behavioral Health Clinic

CODI provides youth integrated behavioral health assessments, recommendations & treatment planning, case management, and individual, group, and family therapy. We also provide community support services including home school and in-school support as well as after school groups. CODI works with children infant through 21 and their families.

What can I do if I need help with my child?

To access our counseling services, you need to call our intake staff to talk about your needs and request an intake packet. You can also download an intake packet, the release of information, or referral form by clicking here and choosing the form you need. You may come to our office (11921 E. Palmer-Wasilla Hwy, next to the light at Hemmer Rd) and pick up an intake packet as well. Once our intake staff receives a completed packet, they will contact you to schedule the next available assessment appointment with the most appropriate clinician based on the information provided to us.

The assessment takes about 2 hours to complete in which the clinician meets with the child and caregiver/guardian to help assess the level of need and diagnosis. However, please be aware that the initial visit is an assessment. Assessment does not guarantee your child will be enrolled in CODI services. You will be informed of a decision to enroll after the assessment has been reviewed by CODI staff. After enrollment, the clinician will work with the family to develop a treatment plan. The clinician and staff will work with the family to schedule appropriate appointments within CODI and possibly other agencies.
